Each year, Employee Benefit Adviser magazine grants annual awards to recognize excellence and innovation in four areas of the field:
- Employee Benefit Adviser of the Year
- Health Plan Adviser of the Year
- Retirement Plan Adviser of the Year
- Voluntary Benefits Adviser of the Year
Nominations for the 2012 Voluntary Benefit Adviser of the Year may be submitted using this form.
Please complete all items requested. Nominations may be submitted via e-mail as well, in the form of an attached Word document including all of the information requested on the form.
