Adviser of the Year nomination form

Each year, Employee Benefit Adviser magazine grants annual awards that recognize excellence and innovation in four areas of the field:

• Employee Benefit Adviser of the Year
• Health Plan Adviser of the Year
• Retirement Plan Adviser of the Year
• Voluntary Benefits Adviser of the Year

Nominations foe the Health Plan, Retirement Plan and Voluntary Benefits Advisers of the Year may be submitted using this form. The winner of the Employee Benefits Adviser of the Year will be chosen from the nominees for the Health, Retirement and Voluntary awards.

Please complete all items requested. Nominations may be submitted via e-mail as well, in the form of an attached Word document including all of the information requested on the form. Supporting information, such as documentation of achievements, letters of support from clients or recommendations from colleagues, are encouraged; please send as e-mail attachments to Elizabeth Galentine.

The deadline for nominations is Friday, June 24. Winners will be notified by July 1.
